I understand that any misrepresentation or omission on this application may preclude an offer of employment, result in a withdrawal of an employment offer, or result in my discharge from employment if I am already employed at the time this misrepresentation or omission is discovered. I understand that Seven One Seven Parking Services, Inc., Hospital Parking Management, and Clean & Park, Inc. contract with third parties to provide services, and that the Seven One Seven Parking Services, Inc., Hospital Parking Management, and Clean & Park, Inc. agreements with these third parties may require Seven One Seven Parking Services, Inc., Hospital Parking Management, or Clean & Park, Inc. to conduct pre-employment and/or ongoing drug tests, criminal background checks, medical examinations, motor vehicle records checks, employment reference checks and/or similar testing or background checks. I understand that, if I receive an offer of employment from Seven One Seven Parking Services, Inc., Hospital Parking Management, or Clean & Park, Inc. such offer may be contingent upon the results of such background checks or testing, and that if I fail or refuse to take the tests or if I refuse to consent to the background checks, I will not be hired. In particular, I understand that Seven One Seven Parking Services, Inc., Hospital Parking Management, or Clean & Park, Inc. intend to administer a post-offer, pre-employment drug screening test in accordance with Florida law. I understand that a positive drug test may disqualify me from employment with each respective Company. I acknowledge that Seven One Seven Parking Services, Inc., Hospital Parking Management, or Clean & Park, Inc. will endeavor to keep the results of any such test confidential to the extent possible and as required by Florida law, but I also understand and agree that Seven One Seven Parking Services, Inc., Hospital Parking Management, or Clean & Park, Inc. may be required to share the results of my background checks or testingwith third parties, and I hereby authorize Seven One Seven Parking Services, Inc., Hospital Parking Management, or Clean & Park, Inc. to release such information to the extent permitted by Florida law.
Notice to Tipped Employees: Pursuant to Section 3(m) of the Fair Labor Standards Act of 1938 (the “Federal Minimum Wage Law”), you are notified that the wage an employer is required to pay a tipped employee shall be an amount equal to-
- The cash wage paid to such employee of not less than $2.13 per hour; and
- An additional amount on account of tips received by such employee which amount is equal to the difference between the wage specified in paragraph (1) and the federal minimum wage in effect (“tip credit”). The additional amount on account of tips may not exceed the value of the tips actually received by an employee. The preceding two sentences shall not apply with respect to any tipped employee unless such employee has been informed by the employer of the provision of this section, and all tips received by such employee have been retained by the employee.

Finally, I understand and agree that, if I am hired, I will be an at-will employee, and that I can quit or can be released at any time for any reason. I understand that no manager, supervisor, or representative of Seven One Seven Parking Services, Inc., Hospital Parking Management, or Clean & Park, Inc., other than an officer of the Company, has any authority to enter into any agreement for employment for any specified period of time or to make any agreement to the contrary to the foregoing. Furthermore, I understand and agree that any such agreement entered into by the officer of the Company will not be enforceable unless it is in writing and signed by me and the company’s duly authorized representative.
